Based in Houston, Texas, Kelly Black-Smith is a dedicated and compassionate Certified Geriatric and Disability Care Manager with a background in Occupational Therapy, business development, and education. With over 25 years of experience across physical rehabilitation, cancer care, home health, and hospice, Kelly has devoted her career to helping individuals and families navigate the aging process with confidence and dignity.

Originally from Louisiana but a proud Texan for more than two decades, Kelly specializes in geriatrics and adult disabilities, ensuring that each client receives personalized guidance tailored to their unique needs. She firmly believes that independence looks different for everyone and that with the right support, people of all ages and abilities can live full, meaningful lives.

Beyond her work in care management, Kelly is an educator, support group leader, and advocate. She is a Professional Member of the Aging Life Care Association and the National Academy of Certified Care Managers, holding active certifications in Occupational Therapy, CPR, and First Aid.

Whether she’s guiding families through difficult decisions, advocating for aging-in-place solutions, or offering compassionate support, Kelly is committed to empowering clients and their loved ones to age with grace, security, and peace of mind.
